And because members order across multiple pharmacies, feedback on compound quality, turnaround times, and consistency aggregates in real time. If quality slips, members shift volume and we know immediately. Cost and quality aren't in tension here — both are optimized through the same network.

Aligning incentives

Most buying groups in the compounding pharmacy space are funded by the pharmacies they recommend. The pharmacies pay vendor fees, referral commissions, or volume-based kickbacks to the group. The group's customer is the pharmacy — not you.

This creates an obvious conflict: the group has a financial incentive to steer your prescriptions to the pharmacy that pays them the most, not the one that gives you the best price or quality. It's the same structural problem that plagues insurance-driven healthcare — the person making the recommendation isn't paying for it, and they may be getting paid to make it.

Wellspring fixes this by flipping the model. We are paid by practices, not pharmacies. Our only revenue is membership fees from the practices we serve. We make money when we deliver savings and quality worth keeping — if we don't, members leave. That's the incentive structure that should exist but usually doesn't.
